A practical reference on Semax: what it is, how it behaves, what the literature reports, and where the honest uncertainties sit.
This page was last updated on 2026-04-15 and is reviewed periodically as new material appears.
The proposed mechanism centres on neurotrophic signalling rather than direct receptor activation. Semax is reported to increase expression of brain-derived neurotrophic factor and nerve growth factor in several brain regions, and to shift the balance between excitatory and inhibitory neurotransmitter systems. Interaction with melanocortin receptors has been suggested because of the parent ACTH fragment. Many of these findings come from rodent studies, and the extent to which they translate to human physiology remains an open question.
Scientific literature on semax is unevenly distributed. A substantial share of published work originates from a small number of laboratories in Russia, while independent replication elsewhere is limited. Human data consist mostly of small trials with short follow-up, and several reported outcomes rely on subjective rating scales. Questions about how much intact peptide reaches the central nervous system after nasal administration, and how long it persists there, are still unresolved. The compound is best described as an active research subject rather than a settled pharmacological agent.
Later generations of the molecule include an N-acetylated form and an amidated form, both marketed online alongside the parent peptide. These variants differ in terminal chemistry and stability, and they are frequently discussed in the same breath even though they have not been compared in controlled trials. Supply outside formal healthcare systems comes largely from laboratories that synthesize peptides to order. Purity and identity of these materials vary widely, and no single body oversees the international trade.
Semax is a synthetic heptapeptide developed in the Soviet Union during the 1980s by researchers at the Institute of Molecular Genetics in Moscow. It was designed as a truncated analog of adrenocorticotropic hormone, retaining only the fragment spanning residues four through ten. Investigators sought a peptide that would preserve the cognitive effects associated with ACTH while eliminating the hormonal stimulation of the adrenal cortex. The compound entered clinical use in Russia during the following decade.

Laboratory work points toward modulation of neurotrophic signaling, particularly expression of brain-derived neurotrophic factor and nerve growth factor in hippocampal tissue. Studies also describe effects on monoamine turnover, inflammatory mediators, and oxidative markers. These observations come mainly from animal models and cultured cells, so the causal chain in humans is not firmly established. Whether the reported molecular changes translate into measurable clinical benefit is an open question. Reviews generally present the mechanism as plausible rather than demonstrated.
Clinical evidence consists largely of small trials with modest sample sizes, often without independent replication. Reported endpoints include cognitive scores, recovery after stroke, and visual function, but study designs vary widely and few trials meet contemporary reporting standards. Systematic reviewers have noted a high risk of bias in several of these reports. No large multicenter trial conducted outside Russia has been published. The compound is therefore best described as investigational in most jurisdictions, with its clinical role still unresolved.
Regulatory status differs sharply between jurisdictions. In Russia the peptide is registered as a prescription nasal preparation, while agencies such as the United States Food and Drug Administration have not approved it for any indication. Products sold elsewhere are typically labeled for laboratory research only, and such labels shift responsibility for safe handling to the purchaser. Because the same name covers pharmaceutical-grade nasal drops and bulk research powder, identity and purity documentation becomes the main practical concern when comparing sources.
Semax is a synthetic seven-amino-acid peptide whose sequence extends the ACTH(4-10) fragment with a C-terminal proline-glycine-proline tripeptide. The commonly cited sequence is Met-Glu-His-Phe-Pro-Gly-Pro, giving a molecular formula near C37H51N9O10S and a molecular weight close to 813.9 g/mol. It belongs to the broader class of synthetic ACTH fragments studied for central nervous system effects rather than for adrenal steroid stimulation. In practice the material appears as a lyophilized white powder for laboratory work or as a dilute saline solution in clinical settings.
The compound was developed in the 1980s at the Institute of Molecular Genetics in Moscow, where it emerged from research on short ACTH fragments and their effects on the central nervous system. Russian pharmaceutical listings describe it as a nootropic and neuroprotective agent, most often formulated as nasal drops. It is not a marketed medicine in the United States or the European Union, and no pharmacopoeial monograph covers it. Consequently, most published clinical experience with the substance originates from a small number of research centres, mainly in Russia and neighbouring countries.
Pharmacological accounts link semax to melanocortin signalling and to modulation of neurotrophic factor expression, particularly brain-derived neurotrophic factor and nerve growth factor. Much of this evidence comes from rodent studies using intranasal delivery, a route chosen because it allows peptides to reach the central nervous system with limited systemic exposure. Whether the same mechanisms operate in humans at comparable magnitude remains an open question. The precise receptor or receptors responsible for the reported behavioural and neuroprotective effects have not been conclusively identified.
Neurolathyrism, is a neurological disease of humans, caused by eating certain legumes of the genus Lathyrus. This disease is mainly associated with the consumption of Lathyrus sativus (also known as grass pea, chickling pea, kesari dal, or almorta) and to a lesser degree with Lathyrus cicera, Lathyrus ochrus and Lathyrus clymenum containing the toxin ODAP. This is not to be confused with osteolathyrism, a different type of lathyrism that affects the connective tissues. Osteolathyrism results from the ingestion of Lathyrus odoratus seeds (sweet peas) and is often referred to as odoratism. It is caused by a different toxin (beta-aminopropionitrile) which affects the linking of collagen, a protein of connective tissues. Another type of lathyrism is angiolathyrism which is similar to osteolathyrism in its effects on connective tissue. However, the blood vessels are affected as opposed to bone.
Despite the acquisitions by big tobacco companies, some independent e-cigarette companies had more success, most notably Juul Labs, as of 2018. As of 2018, 95% of e-cigarettes were made in China. Despite international growth of e-cigarettes during the 2010s, not all regions around the world have yet embraced it as much. In 2018, Indonesia became only one of the first in Asia or the Global South to recognise e-cigarettes as a genuine alternative to smoking tobacco.
14 November RVNAF fighter-bombers supporting ARVN forces at Bu Prang Camp hit both ARVN and PAVN forces in an airstrike killing 20 ARVN and an estimated 95 PAVN. Police used tear gas against protesters marching on the South Vietnamese embassy in Washington D.C. and arrested 30 protesters.
=== Total contact casting === Total contact casting (TCC) is a specially designed cast designed to take the weight of the foot (off-loading) in patients with DFUs. Reducing pressure on the wound by taking the weight of the foot has proven to be very effective in DFU treatment. DFUs are a major factor leading to lower leg amputations among the diabetic population in the US, with 85% of amputations in diabetics being preceded by a DFU. Furthermore, the 5 year post-amputation mortality rate among diabetics is estimated at 45% for those with neuropathic DFUs. TCC has been used for off-loading DFUs in the US since the mid-1960s and is regarded by many practitioners as the "reference standard" for off-loading the bottom surface (sole) of the foot. TCC helps patients maintain their quality of life. By encasing the patient's complete foot — including the toes and lower leg — in a specialist cast to redistribute weight and pressure from the foot to the lower leg during everyday movements, patients can remain mobile. TCC redistributes pressure, protects the wound, letting damaged tissue regenerate and heal. TCC also keeps the ankle from rotating during walking, which helps prevent shearing and twisting forces that can further damage the wound. Effective offloading is a key treatment modality for DFUs, particularly those where there is damage to the nerves in the feet (peripheral neuropathy). Along with infection management and vascular assessment, TCC is a vital aspect to effectively managing DFUs. TCC is the most effective and reliable method for off-loading DFUs.

Freeze-casting can be applied to produce aligned porous structure from diverse building blocks including ceramics, polymers, biomacromolecules, graphene and carbon nanotubes. As long as there are particles that may be rejected by a progressing freezing front, a templated structure is possible. By controlling cooling gradients and the distribution of particles during freeze casting, using various physical means, the orientation of lamellae in obtained freeze cast structures can be controlled to provide improved performance in diverse applied materials. Munch et al. showed that it is possible to control the long-range arrangement and orientation of crystals normal to the growth direction by templating the nucleation surface. This technique works by providing lower energy nucleation sites to control the initial crystal growth and arrangement. The orientation of ice crystals can also be affected by applying electromagnetic fields as was demonstrated in 2010 by Tang et al. in 2012 by Porter et al., and in 2021 by Yin et al. Using specialized setups, researchers have been able to create radially aligned freeze-casts tailored for biomedical applications and filtration or gas separation applications. Inspired by nature, scientists have also been able to use coordinating chemicals and cryopreserved to create remarkably distinctive microstructural architectures.
== Importance == Two pathways appear to exist in L. plantarum for transforming pyruvate into D-lactate. One of them involves the NAD-dependent lactate dehydrogenase that directly produces D-lactate (LdhD), and the other is through the sequential activities of an L-specific lactate dehydrogenase followed by lactate racemase. If the LdhD enzyme is inactivated or inhibited, lactate racemase provides the bacterium with a rescue pathway for the production of D-lactate. This pathway is significant because the production of D-lactate in L. plantarum is linked to the biosynthesis of the cell wall. Mutants lacking LdhD activity that also had the lar operon deleted only produced L-lactate, and peptidoglycan biosynthesis was not able to occur.

== Clinical significance == Mice with mutant carboxypeptidase E, Cpefat, display endocrine disorders like obesity and infertility. In some strains of mice, the fat mutation also causes hyperproinsulinemia in adult male mice, but this is not found in all strains of mice. The obesity and infertility in the Cpefat mice develop with age; young mice (<8 weeks of age) are fertile and have normal body weight. Peptide processing in Cpefat mice is impaired, with a large accumulation of peptides with C-terminal lysine and/or arginine extensions. Levels of the mature forms of peptides are generally reduced in these mice, but not eliminated. It is thought that a related enzyme (carboxypeptidase D) also contributes to neuropeptide processing and gives rise to the mature peptides in the Cpefat mice. Mutations in the CPE gene are not common within the human population, but have been identified. One patient with extreme obesity (Body Mass Index >50) was found to have a mutation that deleted nearly the entire CPE gene. This patient had intellectual disability (inability to read or write) and had abnormal glucose homeostasis, similar to mice lacking CPE activity. In obesity, high levels of circulating free fatty acids have been reported to cause a decrease in the amount of carboxypeptidase E protein in pancreatic beta-cells, leading to beta-cell dysfunction (hyperproinsulinemia) and increased beta-cell apoptosis (via an increase in ER stress).

Pain and stiffness (moderate to severe) in the neck, shoulders, upper arms, thighs, and hips, which inhibits activity, especially in the morning, but which usually persists to some degree throughout the day. Pain can also occur in the groin area and in the buttocks. The pain can be limited to one of these areas as well. It is a disease of the "girdles" meaning shoulder girdle or pelvic girdle. Fatigue and lack of appetite (possibly leading to weight loss) Anemia An overall feeling of illness or flu-like symptoms. Low-grade (mild) fever or abnormal temperature is sometimes present. High or spiking fevers may occur in patients who also have giant cell arteritis. Generalized weakness Note that this is generalized weakness, not muscle weakness. The presence of muscle weakness likely indicates a different diagnosis. Inflammatory swelling and pain of wrists and/or knees (only ~25% of cases) Pitting edema (non-inflammatory swelling) of wrists, ankles, hands, and feet (only ~10% of cases)
Countercurrent Chromatography is a method of separation, that is based on the differential partitioning of analytes between two immiscible liquids using countercurrent or cocurrent flow. Evolving from Craig's Countercurrent Distribution (CCD), the most widely used term and abbreviation is CounterCurrent Chromatography (CCC), in particular when using hydrodynamic CCC instruments. The term partition chromatography is largely a synonymous and predominantly used for hydrostatic CCC instruments.

Semax is based on the ACTH(4-10) fragment, a seven-amino-acid segment of adrenocorticotropic hormone. The synthetic peptide retains the core sequence while removing regions associated with endocrine activity. This modification is intended to isolate effects on the nervous system.
It is registered for clinical use in Russia and a few other countries, typically as a nasal drop formulation. It does not hold approval from the United States Food and Drug Administration or the European Medicines Agency. Outside those markets it is generally handled as a research chemical.
The compound was developed in Moscow and the earliest studies were conducted there, so the primary literature is largely in Russian-language journals. Translation and indexing have been incomplete, which limits access for outside researchers. Independent groups have since published some work, but the total volume remains modest.
It originated at the Institute of Molecular Genetics in Moscow during the 1980s. The work was carried out by a Russian research group that specialized in peptide neuropharmacology.
